Required Documents for Rotators
Residents
- Pennsylvania Medical Training license
- If applicant is from out of Pennsylvania, they will need to obtain a PA Medical Training license, which can take up to six months.
 - Initiated by DIO at LGH upon approval to rotate
 - Requires federal and state clearances, completion of Act 31 training, etc.
 
 - Training Agreement with Institutions
- If an agreement is not in place, can take 3-6 months to process.
 
 - Liability insurance
 - Immunizations
- MMR, Hep B, Varicella, TB
 - COVID and Flu
 
 - Updated CV
 - Photo for ID badge
 - Completion of forms
 - Office of Academic Affairs: Clinical Rotation
- Resident Agreement
 - LGH Confidentiality and Access Agreement
 - Completion of Knowledge Link Modules prior to arriving for rotation
 - Completion of EPIC training class at LGH upon arrival
 
 
Medical students

Students/residents are responsible for obtaining their own housing for the duration of the rotation. Transportation will be required and not provided.
